To be effective, the notification of your claim of copyright infringement should be written and must include the following detail:

1) A verified statement that:
A) You are the owner of the exclusive right you claim has been infringed; or
B) You are authorized to act on behalf of the owner of an exclusive right that has allegedly been infringed.
To be “verified”, the statement must be signed under penalty of perjury, and state that the information in the notification is accurate. Your signature may be electronic.
2) The full identification of the copyrighted work involved.
A statement may include multiple copyrighted works that you claim have been infringed; i.e. you can submit one notification with a list of the allegedly infringed works but each copyrighted work must be fully identified.
The identification of the material that you claim to be infringing must include information reasonably sufficient to permit to locate the material.
3) Contact information
You must provide information sufficient to permit to contact you, including your address, telephone number, fax number and, if available, an electronic mail address. You can alternatively provide contact information for the owner of the exclusive right that you claim has been infringed if you are an authorized agent acting on behalf of the owner.
4) A Good Faith Statement
You must include a statement that you have a good-faith belief that use of the material in the manner complained of is not authorized by the copyright owner, its agent, or the law.
